Download and print the signature page. Collect signatures from the SAC Chair and the SAC Liaison then submit the signature page and the new degree or certificate request to the Curriculum Office via intercampus mail.
Be aware, the submission will not be added to the agenda until the completed signature and supporting materials are received. See submission deadlines for DAC Committee meetings.
