- I need the enrollment verification for Child Support purposes.
-
Child Support requests must be received and completed by Student Records. These forms will be sent by the state to the student. Students must complete and sign their portion of the Child Attending School Confirmation Form before sending it to Student Records. The form will be completed within 3-5 business days of being received at Student Records. Forms cannot be completed until after the drop period. Student Records will then mail the form directly to the state. If you would like a copy of what is sent to the state, please indicate on the form.
Child Attending School Confirmation Forms must be submitted each term.
Additionally each academic year you must have a Written Consent for Disclosure Form on file.
If a parent is requesting an official transcript of their child’s transcript, each request must be accompanied by a copy of the Consent for Disclosure attached to the request. Without the Consent for Disclosure the request will not be completed.
