Design/Print Request Form

• Please review pricing before submitting your order.
• Please complete all the information below.
• Missing or incomplete information may result in a delay or cancellation of your request.
• Please allow at least two weeks in advance for requests. Requests without a specific numerical due date will result in a delay or return of your request.
• Please respond quickly when asked for your approval.
• If you have a design you wish to submit, please email it to us at aspcccomm7.sy@pcc.edu and leave a note under "Comments and specifications" below when filling out this form. You can also drop off a USB Flash Drive at our office in CC101 Monday - Friday 8:00am - 5:00pm. We can accept Adobe PDF, InDesign, Illustrator, or CS4 files, or standard graphics files (.jpg, .png, etc). Feel free to call us at 971-722-4925 or email us at aspcccomm7.sy@pcc.edu with any questions about PDFs or file type information.
