Download the form you need from the above link and open it using MS Word to complete the field(s).
a. Save completed the Request Form and Submit as an email attachment to curriculum@pcc.edu.
b. If needed, attach Related Instruction Form to the same email.
Download and print the matching Signature Page Form and get the appropriate signatures.
Staple signed the Signature Page Form to a hard copy of the Request Form (electronic version has already been sent in Step 2). Send both forms to Curriculum Office, Sylvania Campus, TCB 116 via campus mail.
