Download and print the Signature Page. Collect signatures from the SAC Chair and the SAC Administrator. Staple signed the Signature Page Form to a hard copy of the Request Form. Send both forms to Curriculum Office via intercampus mail.
Be aware, the submission will not be added to the DAC Committee agenda until the completed signature and supporting materials are received.
