Curriculum Forms

The forms on this page are those used by various committees at PCC to review, recommend, change and create new curriculum, degrees and certificates. The forms are to be completed by faculty and processed in accordance with committee and college procedures.

How Do I Use the Forms?

Curriculum forms are filled out and submitted online. A confirmation and signature page will be sent via e-mail after processing. Degree and certificate forms must be printed and submitted in order to initiate the process. If you need assistance with any forms or have questions, please feel free to contact the curriculum office.

Note: To our beloved Macintosh users: Mac's are great, BUT... SAFARI has proven to be difficult to use as a browser to complete curriculum forms. You are encouraged to use Mozilla Firefox when possible.

Which Form Do I Use?

Review the list below and choose the form that aligns with your request. Still unsure -- try the Quick Guide to curriculum requests. Many curricular revisions also require a revision to degrees or certificates this includes a change in credit hours, course title, course number, adding a new course or inactivating a course within a degree or certificate.

You'll notice new course forms! Tell us how they work for you, we welcome feedback.
Anticipate a minimum of three months or two terms before a course is available to be scheduled and students enrolled.

Degree & Certificate Forms

Anticipate a minimum of eight months to complete the approval process for a NEW degree and/or certificate and a minimum of four months to complete approval process for a REVISED degree and/or certificate.
