Curriculum Forms

The forms on this page are those used by various committees and processes at PCC to review, recommend, change and create new curriculum. They are completed by faculty and processed in accordance with committee and college procedures. For further information on the procedures, contact the curriculum office or the Curriculum Committee or Degrees and Certificates Committee.

How Do I Use the Forms?

Most forms are filled out and submitted online. A confirmation and signature page will be sent via email after manual processing by the curriculum office. Degree and Certificate forms require printing and sending hard copies to initiate the process. If you need assistance with any forms or have questions, please feel free to contact the curriculum office!

Note: To our beloved Macintosh users: Mac's are great, BUT... SAFARI has proven to be difficult to use as a browser to complete Curriculum Forms. You are encouraged to use Internet Explorer when possible.
