Open Enrollment 2008-2009
The College’s medical, vision and dental plans are changing (See Key Changes This Year) October 1, 2008 as a result of joining the Oregon Educators Benefits Board (OEBB). Please review the following information carefully along with the packet that will be mailed to you.

Everyone that is benefits eligible must complete an enrollment form for medical, dental and vision coverage – either online or paper. See How to Enroll or Opt Out. Please note: Part-time faculty are not eligible for dental coverage. See Part-Time Faculty Eligibility for more information.

Employees who do not enroll will be assigned to the College’s designated default plan (ODS Plan 8) for employee only coverage. No vision and no dental plan will be assigned. You will not be able to make changes until next year’s open enrollment, or within 30 days of a qualifying event.

All non part-time faculty who have other group coverage may opt out (formerly called “waive”) of the PCC plans and receive $200 in lieu of the College’s coverage, the incentive payment will be taxed. Part-time (non-faculty) employees receive a prorated amount based on their full time equivalency (FTE). Part-time faculty members who have either other group or individual coverage may opt out of the PCC plans. There is no compensation for part-time faculty members who opt out. You must let us know of your other coverage by enrolling on-line or on paper; otherwise you will be assigned to the College’s designated default plan.

Review your life insurance needs and make any necessary changes to your current voluntary coverage. If you do not already have voluntary life or AD&D, you may apply for coverage at any time during the year with approval from UnumProvident. You will need to submit a UnumProvident enrollment form to request changes to current coverage or to enroll for the first time, and may need to submit an evidence of insurability form. Review your long term care insurance needs. If you are currently enrolled you may request to increase your daily benefit. If you are not currently enrolled, you may apply for coverage at any time during the year with approval from CNA. See CNA Long Term Care Insurance for more information.
- Flexible spending account open enrollment will occur in November and will be effective January 2, 2009. Information will be sent to all eligible employees in November.
Please review your insurance options and enroll online by 5pm on September 15, 2008. Paper enrollment forms must also be submitted to Human Resources, CA SSB 300, by this deadline.
Every effort has been made to report information accurately. However, all information, including the amount of any benefit and employee eligibility for benefits, is subject to and governed by the terms and conditions of the applicable contract, policy or plan document. In all cases where any of the information provided in this guide differs from the amount of benefit actually provided, the terms of the legal documents will control. Employees are encouraged to check their October 1 check to ensure any payroll deductions reflect the enrollment decisions you have made. Please notify Benefits ASAP if anything appears to be incorrect.

- Open Enrollment begins - Everyone must complete a form - either online or paper. See Steps to Enroll.
- September 15, 2008
- PCC In-Service - last day to enroll
