This form is used by Division Deans or Department Chairs. Please review the four timeline options below prior to filling out the request to ensure that instructors will be able to complete training and course development by the term they are scheduled to teach the course.

Orientation:Participating instructors with a Letter of Agreement are eligible to submit up to 20 hours of training time. Hours are paid at the current Special Project rate.
Compensation: New course: 100% stipend (credit hours x current DLDEV rate = stipend)
Major Revision:
up to 50% of new course stipend
Take Over/CLWEB:
No stipend

After the Distance Learning Department has received your request, the request will be evaluated to see if the course should proceed into the development process. Once the request has been approved by Distance Learning a Letter of Agreement will be created and sent to the Division Dean for signatures.
